Implements outcome-based scoring that automatically ranks memories by effectiveness, with platform-specific delivery: Claude Code uses hooks to inject context and prompt the main LLM for scoring, while OpenCode uses an independent sidecar model to eliminate self-assessment bias. Organizes persistent memory across five collections (working, history, patterns, memory_bank, books) with automatic context injection before each AI response, plus MCP tools for search, manual updates, and document ingestion. Designed as a drop-in MCP server for Claude Code and OpenCode that requires only two setup commands and zero workflow changes.
